How long are the appointments?
Your first appointment will be between 60-90 minutes long. Pediatric new patient visits (under 12 years old) will be 45-60 minutes long. Follow up appointments will be 20 minutes long.
How do I sign up for an appointment?
Book an appointment with us by clicking the "book now" button. Once you follow the prompts and fill out your information, you will be sent an email that contains your new patient paperwork. Please fill this out before your first visit.

After scheduling your initial appoitment you will receive an email with a link to paperwork that you will need to fill out.

Do you take insurance?
We do not take insurance. If you would like to submit your visit to an insurance company, we will happily give you a superbill that you can give to your insurance company.
How much do appointments cost?
A new patient appointment costs $200.
A pediatric (under 12) new patient appointment costs $150.
A follow up appointment costs $90.
What is your cancellation policy?
Appointments need to be canceled with at least 24 business hours notice. Any cancellations after this time will be charged for their appointment.

Can I use my HSA?
Most HSAs will be covered, however, check with your HSA provider for more details ​
Where should I park?
Parking is available behind the building. Turn East on 2nd St. SW and the parking lot will be your first left.

Please park near the far north building of the parking lot. This is the State Farm building and the clinic is inside.
